Complete the top section of the form only. The bottom half of the form will complete itself.
Sign and date the top half of the form. Submit the form to your division head or supervisor for their approval and submit the completed absence report form to the Payroll Office no later than the 10th of the month following the absence.
Date and retain the bottom half of the form for your own records. You may wish to have you division head or supervisor initial this as a record of the approval.
